Certificate of Tax Exemption/Certificate of Indigency

Photocopy of Form 138 (Report Card) for incoming freshmen/ Photocopy of Recent Grades from the Previous Semester for old students and transferees

Medical Certificate Must have taken laboratory tests on CBC and Urinalysis and Chest X-Ray

Letter of Consent signed by the Parent/Guardian

Duly signed Data Privacy Form Available at the Office of Student Affairs / Office of Safety and Security
